Participant Consent Form

Thank you for your interest in the QESLI. We’d like to provide you with information about the research project. Your participation in the study is voluntary, and there is no penalty for refusing or discontinuing participation.

In this study, you will respond to a survey by answering questions related to your wellbeing and school experience. The risks associated with the study are minimal. If you feel uncomfortable answering questions about yourself, you have the right to skip any question.

Your assessment answers will be used to collect data regarding your the quality of education and student life at your school. All personal information gathered in this study will remain confidential and be given out only with your permission or as required by law. You will be assigned to create a random participation code to take the survey.
